To enroll, please contact the Mayfair Membership Chair for application submittal instructions at mayfairpreschool@gmail.com or call at (248) 626-2759.   Applications will be processed on a first come, first served basis.

Health Appraisal Form*

Please fill out section I of this form. Then, give this form to your child’s physician. They will fill out section II and provide you with a copy of your child’s vaccination records.* The Health Record Form and Vaccination records will be collected at the August Parent Group Meeting/Orientation.

*NOTE:  If your child is not up to date on his/her vaccinations, you will be required to provide either a medical waiver from your doctor or a vaccination waiver from the Oakland County Health Department.
